Policy: The California Plan
If you've been listening to NPR, reading the NYT or the WSJ - you've been inundated with information about California's new health initiative.
In short, the plan would (WSJ, January 9):
1) Mandate coverage for all Californians,
2) Employers with more than 10 workers who don't provide medical coverage would pay 4% of their payroll into a state fund to offset costs to the State,
3) The plan would levy a tax of 2% on doctors and 4% on hospitals to further fund the State's insurance plan.
The only problem is we still don't know how much the program will cost, how they will cover the large number of illegal immigrants in the region, and how - exactly - this will ensure costs are contained over the long run. Given the major supporter of the program is Blue Cross of California...who called the plan "bold and visionary"...I think it's fair to say that insurers plan to do pretty well in the deal...unlike the providers being asked to foot part of the bill.
So who's got a reasonable plan underway?
I still think Tennessee may lead the pack (with the Governor a former Health Plan Exec)...and Vermont as a sleeper pick. Any others out there?
I'm still looking for a plan that deals with controlling costs and helping insure the uninsured...

Any idea how California's proposal handles enforcement and subsidy?
There are some aspects that I don't understand. Why tag health care providers to help foot the bill? And if they do, won't the hospitals and physicians just increase prices to build a historical base to justify the next set of UCR/R&C COLA adjustments?
What I would be interested in seeing is figures on the risk adjusted premium if you threw every single body in the state into the pool.
Isn't it a shame, though, that our national government refuses to participate in solving national problems, forcing the states to try?
Things like health care, education and global warming so clearly need to be addressed on a national and even international basis...

An effective way to reduce the number of uninsured is to address the over-insurance of those that have insurance. A key reason most industrialized countries that have universal health coverage spend less on coverage AND care, is that over-insurance (i.e too much insurance, no incentives to spend wisely) doesn't exist. Rather, they maintain a scope of benefit package which allows ALL to be insured, affordably.

We are looking to recruit docs and this will be one more reason for them not to look into or remain practicing in California. The idea that the docs will make more money because everyone will be covered might be valid, if docs were not treadmilling to the max already.
The more you tax something, the less you get of it, I hope the last practicing phyisican in California turns out the light
